Move to Value Virtual Summit
October 14, 2021 10:00 A.M. – 3:00 P.M.
Where we are born, live, learn, work, play, worship, and age all greatly affect our health and quality of life. These factors, also known as Social Determinants of Health (SDOH), often have a greater impact on health outcomes than the actual delivery of health services. Despite SDOH’s prominent role in health outcomes, health care traditionally has failed to address these non-medical conditions due to misaligned incentives and lack of resources. Value-based care is paving the way to improving health outcomes by providing care that acknowledges and addresses SDOH. This holistic care not only improves the overall health of a population and assists in achieving health equity, but also leads to a decrease in health care costs and a significant return on investment.
Join us on October 14th for the Autumn session of the Move to Value Summit to learn about the domains of SDOH, its role in addressing health equities, the challenges remaining to address these conditions, and how to apply this key competency to succeed in value-based care.
The Move to Value Summit is designed to engage and inform Health Care Providers and Professionals who work in Contracting, Network Management, Provider Engagement, Provider Relations, Compliance, Care Teams, Quality, and Finance within Hospital Systems, ACO’s, Physician Groups, Clinically Integrated Networks, Independent Practice Associations (IPAs), Health Plans and Payors, and Employer Groups.
