Clinical Documentation is more important than it has ever been. Accuracy and specificity are of upmost importance as we continue down the road of value-based healthcare. This presentation will include an explanation of the CMS-HCC Risk Adjustment Model, details of ICD-10/ HCC coding specificity, clinical examples and an explanation of the impact this has on RAF score calculation. It will include a discussion of the role of coding experts (CDI Specialists and certified coders) in supporting providers.
