Value-Based Contracting

Value based contracts reward work that improves quality and lowers the cost of care. We collaborate with value partners to develop a contracting strategy based on goals and readiness for value. These contracts are negotiated with the understanding of quality measures, targets, and requirements of providers and healthcare systems. This has resulted in financial success in both MSSP Shared Savings Programs and ACO REACH, as well as Medicare Advantage and commercial contracts.

Care Models

We work closely with value partners to implement innovative care models that address the needs of the sickest patients, while aligning clinical and financial goals. CHESS Care Models provide in-depth programs that include care coordination, pharmacy services, and comprehensive organizational education.

Population Health Analytics

The combining of clinical electronic health record (EHR) data with patient claims data provides a view of the health of an entire patient population. We can then identify major cost and quality drivers for given populations, assess risk of future adverse health outcomes, deliver provider level details, and study quality and outcome performance. The analytics package includes regularly scheduled meetings with value partners to review reports and provide actionable information.

Continuous Operational Improvement and Patient Engagement

After implementing care transformation, we continue to assist value partners in maintaining the momentum toward value. Regularly scheduled meetings between the value partner and a designated operations team focus on driving utilization, quality, and patient satisfaction across all value contracts. The continual improvement process also uses shared learning opportunities and the Plan-Do-Study-Act (PDSA) cycle to prepare value partners for safely taking on downside risk contracts.
