Clinical Documentation is more important than it has ever been. Accuracy and specificity are of upmost importance as we continue down the road of value-based healthcare. This presentation will include an explanation of the CMS-HCC Risk Adjustment Model, details of ICD-10/ HCC coding specificity, clinical examples and an explanation of the impact this has on RAF score calculation. It will include a discussion of the role of coding experts (CDI Specialists and certified coders) in supporting providers. Dr. Lennon will convince you that your documentation is the foundation for excellent communication regarding patient care, accurate billing, meeting compliance requirements, Risk Adjustment calculations, and Quality measures.
